In a League of Their Own: Safariland 'Safety' Holsters

I've been collecting the published reports of gun/holster safety incidents for some years now, and they tend to be Safariland related. Perhaps because that company dominates the LEO market? And uniformed officers' incidents are (a) likely to be reported and (b) likely to be litigated.
